In addition to the parts above, you will also need several short pieces of wire, electrical tape and/or heat shrink tubing.  I used three 4" pieces in two different colors.  I used black for the ground, and another for the signal wires.

Solder one end of each resistor to a non-stripped end of each diode.  Insulate the bare metal at this solder joint.

Solder each stripped end of the diode and a 4 inch piece of black wire together.  Insulate all bare metal at this solder joint too. 

Solder a 4 inch piece of (any color but black) wire to the open end of each resistor, and insulate all bare metal.  Insulate entire assembly using electrical tape or, for a more professional job, use heavy duty heat shrink tubing. 

Connect the ground (black) wire to ground, and one of the other wires to the left signal wire.  The last wire will connect to the right signal wire. 

When connecting signal wires, I just stripped a small amount of insulation from the signal wires, soldered the load equalizers to them, and reinsulated the connections.
